- Identification of the material on this site said to be infringing, with the page address.
- Your address, telephone number and email address.
- A statement that you believe in good faith that the use is not authorized by the owner, its agent or the law.
- A statement that the information in the notice is accurate and, under penalty of perjury, that you are authorized to act for the owner.
Valid notices are acted on promptly, and the material is removed or disabled while the claim is reviewed.
Counter-notice
If material you provided was removed and you believe that was a mistake, you may send a counter-notice with the same details, a statement under penalty of perjury that the removal was a mistake, and your consent to the jurisdiction of the federal courts in your district.
